Create a new scorecard using the official site list. The dashboard will attach the score to the right quarter automatically from the visit date.
Use the standard service line structure below. The first three labels stay fixed, and the fourth can remain Other or be customized for the visit.
For each criterion, score the applicable service lines and add notes where helpful.
